Need help putting contents from a PDF into excel format - Upwork

I need someone to take the contact information beginning on page 43 of the attached PDF through page 104, and put it into Excel format. I only need to see the company name, their category (ex: Retail, Flor, GH), the individual person's first and last name (below the category), and the individual person's email address.
The first listing in the PDF on page 43 is 14th Street Garden Center. The data I'd want to see is:
- Company Name (Column): 14th St Garden Center
- Category (Column): Retail, Flor, IP
- Individual First Name (Column): Vincent
- Individual Last Name (Column): Giordano
- Individual email (Column): Vince14stgardencenter@yahoo.com

The second listing of 1st Farm Credit Services would be:
- Company Name (Column): 1st Farm Credit Services
- Category (Column): Sup_Serv,
- Individual First Name (Column): Mark
- Individual Last Name (Column): Ringhouse
- Individual email (Column): Mringho@1stfarmcredit.com
